Categories

Moving from a Regulated Province

A Canadian Certified MLT applicant moving from a Regulated Province, in accordance with Canadian Free Trade Agreement, Chapter 7.

Documents required:

•  Evidence of Certification - CSMLS Certificate

•  Evidence of Education - University or College Diploma or Degree (Official transcripts may be accepted)

•  Evidence of Membership - CSMLS member card (if applicable)

• Letter of Standing from current regulatory body- Applicant must request letter be sent directly to SSMLT

• Quebec certificate (if applicable)

• Quebec member card (if applicable)

• For Renewal the member must provide the following:

     -evidence of Continuing Professional Education (within the previous 5 years)

     -evidence of Practice hours (within the previous 5 years)

Moving from a Non-Regulated Province

A Canadian Certified MLT applicant moving from a Non-Regulated Province.

Documents required:

• Evidence of Certification - CSMLS Certificate

• Evidence of Education - University or College Diploma or Degree (Official transcripts may be accepted)

• Evidence of Membership - CSMLS member card (if applicable)

• Letter of Recommendation for Provincial Association

• Quebec certificate (if applicable)

• Quebec member card (if applicable)

• Evidence of Continuing Professional Education (within the previous 5 years)

• Evidence of Practice hours (within the previous 5 years)

New Graduate Certified

A New Graduate Certified applicant who successfully challenged the Canadian certification exam.

Documents required:

•  Evidence of Certification - CSMLS Certificate

• Evidence of Education - University or College Diploma or Degree (Official transcripts may be accepted)

• Evidence of Membership - CSMLS member card (if application)

• New graduates have 5 years to obtain the required Continuing Professional Education and Practice hours

Internationally Educated MLT (IEMLT)

An Internationally Educated Medical Laboratory Technologists Certified who successfully challenged the Canadian certification exam but has not worked in Canada.

Documents must be reviewed by SSMLT office to determine application eligibility.

Documents required include:

• Evidence of Certification - CSMLS Certificate

• Evidence of Education - CSMLS Prior Learning Assessment

• Evidence of Membership - CSMLS member card (if applicable)

• Evidence of Continuing Professional Education (within the previous 5 years)

• Evidence of Practice hours (within the previous 5 years)

How to Apply

Complete the Certified MLT Application Form, remit fees and submit required documents.

Requirements

  • Enter all required profile information.
  • Pay applicable application and registration fees.
  • Enter your Practice Hours information through your "Applicant Portal"
  • Enter your Continuing Education through your "Applicant Portal"
  • Upload your supporting documents through your "Applicant Portal"

The on-line application will allow you to create your own login-id and password. These login credentials which you may reset, will allow you access to your ‘Applicant Portal’ where you can enter in your Continuing Education, Practice Hours and upload the required supporting documents.

Please note that once you submit your on-line application form, your application formembership will be reviewed and will have a status of ‘pending’. SSMLT staff will review your application for membership and either approve it or contact you should further information be required.
